New Patient Registration
New Registrations are dealt with between 9am and 6pm
The practice accepts registrations from patients living in:

The doctors welcome new patients who live within our practice area. As it often takes sometime for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire.
Please collect a new registration pack from the reception desk. Once you have completed the relevant forms you will be offered an appointment Monday - Friday between 11.30 am and 12.30 pm or 5pm and 6pm to return your completed forms and bring your proof of address and identity to us
Adults > 16yrs.
Photographic ID e.g. Passport, driving licience
Proof of address dated within the last 3 months e.g. Utility bill, council tax bill, rental agreement
All adults will be asked to take their blood pressure on our machine in the reception area and book an appointment for a new patient check.
PLEASE NOTE WE MUST SEE EACH ADULT WITH THEIR PHOTOGRAPHIC ID
Children under 16yrs.
