As a result of this update, you may see an entry in your medical record that has the wording “complex health needs”, or a code to say that your health is “generally good”.
Please do not worry regarding this wording. This code on your record is to help us serve our patients better, according to their health needs. You do not need to do anything differently, and the code does not mean your health has changed.
